BIRTH CERTIFICATE REQUESTS

Requests for Birth Certificates require filling out and submitting a BIRTH CERTIFICATE REQUEST (available below for download), along with the required identification materials to receive birth certificates by mail from the Boone County Clerk. Fill out the request form completely and send it along with the $7.00 fee ($7.00 for first copy, $2.00 for each additional copy), and a self-addressed, stamped envelope to:

Personal checks from the State of Illinois only are accepted.
Otherwise, certified check or money order is required.

MARRIAGE LICENSE INFORMATION

WHERE TO APPLY:
Both parties must appear in the office between the hours of 8:30 a.am. and 4:30 p.m. Monday thru Friday.

PROOF OF AGE: & REQUIREMENTS:
Must present their birth certificate if 21 years of age or younger.
Both parties must be 18 years of age to obtain a license.
If under 18 and not younger than 16 years of age, must have parents' consent (requires parents to be present to sign).
If a previous marriage ended in divorce, you will need to bring the divorce papers showing the Judge's signature.

PROOF ID:
Photo ID is required for both parties, such as a driver's license.

MEDICAL EXAMINATION:
The State of Illinois does not require.

NON-RESIDENTS:
Non-residents CANNOT obtain a marriage license if the said marriage would be void in their state.

VALID:
Marriage license is valid after a one day waiting period and then it is good for 60 days.
Persons obtaining license in Boone County MUST be married in Boone County.

VOTER REGISTRATION - WHO IS ELIGIBLE TO VOTE:

To be eligible to register to vote, a person must be a U.S. Citizen; be 18 years of age as of the next election and reside in the precinct for 30 days prior to the next election. Naturalized citizens must provide the name of the court where naturalized, state and date of naturalization.
